I found the report very detailed, coupled with the submissions at the end of the report it gave me a much greater understanding of this tragic incident. Maybe the best thing we can do is understand how it happened and hope to avoid such a thing happening again. It is with this in mind that I would ask all paddlers to read this document.

In relation to this conclusion of the report:

The carriage of mobile phones in suitable watertight covers, by kayakers, for use
to summon assistance in emergency situations should be considered.

For anyone interested in getting one of these, I bought this a few weeks back and its savage for something so cheap - you can make and receive calls with it still in the pouch. Easily fits in a BA front pocket.
